I have read and agree to the MBYSF Participation Terms, including the Assumption of Risk, Release of Liability, and Emergency Medical Authorization, Photo/Media Consent and acknowledge Flag Lab as the independent coaching provider for this program.*

I give permission for my child to walk home without adult supervision upon dismissal from the program. I understand my childs safety is not the responsibility of Miami Beach Youth Sports Foundation or the coaches, once my child has left the training field.
